Medical Record Review

Description

After obtaining your medical records from hospitals and clinics where you have received care, we will thoroughly review your history and write a comprehensive summary explaining in plain English what has happened to you and how it weighs on you currently. I will follow that up with a phone conversation to answer your questions. We will also make recommendations moving forward. Additional charges will apply for medical records exceeding 1,000 pages. This process can take some time, mostly to obtain the original records but once in hand, we’ll take two weeks to deliver the report.
